1. The form height needed for each part of the job.
2. The number of individual 10-foot straight sections at each height.
3. The number of individual 10-foot flexible sections needed for curved runs.
4. The number of matching 90-degree corners and 5-foot fillers.
5. Pickup or freight; for freight, have the complete delivery address ready.

Choose the parts

Four products. Four jobs.

Straight forms: 10-foot sections for straight runs.

Flexible forms: 10-foot sections for curves and radiuses.

Corners: 90-degree corners made for the female end of Ohio Metal Forms straight sections.

Fillers: 5-foot sections that slide over straight forms to adjust the length of a run.

How to read a size: 6" × 10' means a 6-inch form height and one 10-foot-long section. Quantity 3 means three individual sections.

Match the heights: Choose fillers and corners with the same listed height as the Ohio Metal Forms straight sections they connect to.

What is added: Each quantity adds one listed form piece. Pins, stakes, and other jobsite hardware are not automatically added; call before paying if you need them.

Pickup or delivery

Know how payment works.

Production time: Most forms are made to order. Typical production time is about 7–8 business days, with the exact ready date confirmed after order review.

Picking up: Pickup is Monday–Friday, 8:00 a.m.–4:30 p.m. Choose local pickup at checkout, review the order, and pay securely online. Wait for the ready-for-pickup email and instructions before arriving.

Need freight: Choose freight delivery and enter the complete delivery address. We add the exact freight charge, then email a private link showing the products, freight, applicable tax, and full total. You pay after you review it.
